To enroll, please gather the requested documents and information below and complete the enrollment form for your student's school site.

    *NOTE: Please register your student at the school site in the district identified by searching your home address in the Find My School app.

Required School Enrollment Documents

  • Name and Address of Your Student's Previous School (if applicable) Please provide name and address of your student's previous school so we can assist in obtaining student records for enrollment.
  • Suggested Health Check Health and wellness are critical in setting your student up for success in the classroom. Please review and discuss with your family healthcare provider the suggested Health Check for students.
  • Student's Immunization Record & Requirements Click here to review a list of immunizations that must be obtained to comply with requirements of the West Virginia Bureau of Public Health for enrollment, unless medically exempted..
  • Copy of Student's State Issued Birth Certificate Hospital or County Birth Certificates are not acceptable. Certified Birth Certificates must be obtained from your State's Office of Vital Statistics.
  • West Virginia contact: Vital Registration,  Room 165, 350 Capitol Street, Charleston, WV 25301-3701 or (304) 558-2931. The fee is $12.00; click here for a copy of the application. Please allow several weeks prior to request for receipt of certificate.

  • Copy of Student's Social Security Card (optional) or Social Security # (required) Please bring your student's social security card for verification during your enrollment appointment.
  • Proof of Residency for Berkeley County, WV Please provide a copy of one of the following with name/address: Utility bill, Mortgage, Rent agreement, etc.
