Immunization/Health Reminders

November 2018 

Dear Parents/Guardians of Current 1st grade, 6th grade and/or 11th grade students:

The WV Department of Education and the WV Department of Health and Human Resources Bureau for Public Health have updated school entry requirements to minimize health barriers to attendance and learning.  They follow current recommendations from the US Center for Disease Control (CDC).

All students entering 2nd grade in Fall 2019 are requested to have:

  • Health Check / Physical exam (completed within prior 12 calendar months)
  • Dental exam (completed within prior 12 calendar months)
  • Turn in documentation of Health Check and dental exam to your child’s school prior to the first day of school for students, August 2019

All students entering 7th grade in Fall 2019:

  • Must have one dose of the Tdap (Tetanus, Diphtheria, and Pertussis) vaccine
  • Must have one dose of the MCV (Meningococcal) vaccine
  • Turn in copy/proof of required vaccines to your child’s school by Friday, May 17, 2019
  • Health Check/ Physical exam (completed within prior 12 calendar months)
  • Dental exam (completed within prior 12 calendar months)
  • Turn in documentation of Health Check and dental exam to your child’s school prior to the first day of school for students, August 2019

All students entering 12th grade in Fall 2019:

  • Must also have had at least one dose of the Tdap vaccine
  • Must have second dose of the Meningococcal vaccine. (If the first dose of the Meningococcal vaccine was received after the age 16, only one dose of this vaccine is required.) 
  • Turn in copy/proof of required vaccines to your child’s school by Friday, May 17, 2019
  • Health Check/ Physical exam (completed within prior 12 calendar months)
  • Dental exam (completed within prior 12 calendar months)
  • Turn in documentation of Health Check and dental exam to your child’s school prior to the first day of school for students, August 2019

Please call today to schedule an appointment with your child’s physician or with a local health department to have your child vaccinated so that he or she can attend the first day of class next fall.  Berkeley County HD- 304.263.5131, Jefferson County HD- 304.728.8416, Morgan County HD- 304.258.1513.
