I/DD Waiver Application Information

The ?I/DD Waiver Program provides services that instruct, train, support, supervise, and assist individuals who have intellectual disabilities and/or developmental disabilities in achieving the highest level of independence and self-sufficiency possible in their lives. The  I/DD Waiver Program provides services in natural settings, homes and communities where the member resides, works, and shops instead of ICF/MR facilities.    
 
To apply for Waiver, follow the steps below:  
 
  • Fill out one-page application for I/DD Waiver (click here for form) .
  • Fax or mail application to KePro which is the Waiver administrator for WV.
  • KePro will send you a list of approved professionals to do a psychological exam.  You schedule appointment and have individual evaluated.
  • Within 45 days, you will be notified whether individual qualifies for Waiver based on psychological exam findings.  If so, he/she will be added to wait list, now estimated to be 7-year wait.
  • You will be notified when slot becomes available.
