Required School Registration Documents
Suggested Health Check for Kindergarten
Health and wellness are critical in setting your student up for success in the classroom. Please review and discuss with your family healthcare provider the suggested Health Check for Kindergarten students.
Proof of Residency for Berkeley County
- Please provide a copy of one of the following with name/address:
Utility bill
Mortgage
Rent agreement
Name and Address of Your Student's Previous School (if applicable)
- Please provide name and address of your student's previous school so we can assist in obtaining student records for enrollment.
Tuberculin Skin Testing (PPD)
- As of May 25, 2015, Tuberculin Skin testing (PPD) is no longer required for students entering from out of state. However, students entering from a country with high risk will be required to complete a PPD.
- Please review this list of those countries recommended by the Berkeley County Health Dept for which a PPD test will be required before attending school.
Immunization Requirements
- REQUIREMENTS for students entering school in WV for first time, grades K – 12, unless medically exempted

Social Security Card
- Please bring your student's social security card for verification during your enrollment appointment.
Certified Birth Certificate
- Hospital or County Birth Certificates are not acceptable. Certified Birth Certificates must be obtained from your State's Office of Vital Statistics. West Virginia contact:

Medical & Special Diet Forms
All medications, including over the counter medicines and special diet requirements must have a medical order on file.
