All student-athletes MUST have a sports physical turned into the office before the first day of tryouts. This is different from an annual check up.

The four-page WVSSAC and BCS Sports Physical packet must be turned in to the front office before a student-athlete can attend tryouts or practices. Physicals are not required for conditioning or FLEX days. The form can be downloaded below or picked up from the main office. All local Urgent Care facilities do Sports Physicals without appointments for $25. .

 If your student-athlete has a physical form on file with SMHS they need to message Ms. Pavlik (maryjean.pavlik@k12.wv.us) and ask for it to be transferred to our school for their sports season with us.
